MEDICAL INSURANCE

Information regarding medical insurance coverage applies to all covered employees and dependents.

  Accepted at participating pharmacies Generic: $10.00

Doctors/Hospitals

You may visit PPO Providers/Hospitals or the Doctor/Hospital of your choice.

PPO Provider & Hospital

  • Doctor's Office Visit - $15.
  • Co-insurance at 90%.
  • No charges above Usual & Customary.

Non-PPO Provider

  • All visits and services are applied to the deductible.
  • Co-insurance at 80% of Usual & Customary.
  • Other PPO & HMO plans are available. Please contact Human Resources for more info.

LIFE INSURANCE

  • Non-Exempt Positions - $30,000.
  • Exempt Positions - $50,000.

Voluntary Supplemental Life Insurance

All employees have the option to purchase $50,000 of Supplemental Life Insurance, offered by Standard of Oregon Insurance. Supplemental Life Insurance is also available starting at $10,000 for spouse and $2,000 for children.

DENTAL INSURANCE

Choose any dentist you prefer.

  • $50 Calendar year deductible, maximum $150 per family.
  • Preventive care includes two cleanings and one check-up per calendar year, prior to meeting deductible.
  • Most services covered at 80%.
  • Major work, i.e. crowns and bridges, covered at 50%, subject to preauthorization.

VISION

  • One exam per calendar year covered at 100%.
  • Optometrist or Opthmalogist of your choice.
  • $40 towards eyeglass lenses, $40 towards frames, OR $80 towards contact lenses every 12 months.

VOLUNTARY SHORT-TERM DISABILITY

Voluntary Short-Term Disability Insurance is available through Prudential Insurance for total disability. Coverage is 70% of weekly base earnings to a maximum of $1000 per week for up to 26 weeks.

PROFIT SHARING/401(k)

Employees who meet eligibility requirements will be enrolled in the Profit Sharing Plan and may elect to enroll in the 401(k) Retirement Plan.

Based on annual profits, discretionary funds are disbursed into the Profit Sharing Plan and allocated to all eligible employees' accounts based on annual compensation.

Employees may elect to contribute pre-tax wages up to the annual IRS maximum to Ryan Herco's 401(k) Retirement Plan. Ryan Herco matches 50% of employee contributions up to a maximum of 4% of employee earnings. Employees may direct the investments of their accounts among several investment funds.

Vesting is based on number of years of employment:

  • 20% -- 2 years of employment.
  • 40% -- 3 years of employment.
  • 60% -- 4 years of employment.
  • 80% -- 5 years of employment.
  • 100% -- 6 years of employment.

Employees are always 100% vested in their own contributions.
